Radiologist Cuomo deems the war on cancer a stalemate at best. Her opening sentence is a salvo: “The system designed to study, diagnose, and treat cancer in the United States is fatally flawed.” She believes the expectations of actually curing the disease have been replaced by a clinical resignation to living with it as a chronic illness along the lines of diabetes or hypertension. She’s not happy with the triad of treatment options for cancer—surgery, chemotherapy, and radiation, satirized as “cut, poison, and burn.” Cuomo finds fault with their toxicity, expense, and often minimally beneficial long-term results for individuals with advanced malignancies. She cites one study showing that cancer patients who received palliative care survived about three months longer, with less depression, than those who obtained standard therapies. She offers recommendations emphasizing prevention. Change behaviors linked to a raised risk of cancer. Lower exposure to medical radiation and environmental contaminants. About the Author
Margaret I. Cuomo, MD, is a board-certified radiologist and served as an attending physician in diagnostic radiology at North Shore University Hospital in Manhasset New York for many years. Specializing in body imaging, involving CT, Ultrasound, MRI and interventional procedures, much of her practice was dedicated to the diagnosis of cancer and AIDS. She is the daughter of former New York Governor Mario Cuomo and Mrs. Matilda Cuomo and sister to Governor Andrew Cuomo and ABC's Chris Cuomo. She resides in New York.

Easy & smart changes I am making.

"A World Without Cancer" has given me a list of simple changes I am beginning to make to reduce my exposure to harmful chemicals in my personal grooming & cleaning products, and in my lifestyle choices. From eliminating shampoos, creams, and makeup with four harmful additives, to switching to dry cleaning that doesn't use harmful chemicals (surprisingly easy to find), to storing my foods in glass containers, I am viewing the many suggestions as an ongoing project in my life for myself and my family. Dr Cuomo offers suggestions for diet, including specific ingredients, water, supplements, medical testing, cell phone use, and medicines, just to name a few. This book is invaluable, and I expect I will be referring to it for many years to come. Thank you, Dr. Cuomo for being an advocate for us all.

Radiologist and author Cuomo reports that 41-years and $90 billion dollars into the War on Cancer were still seeing over 1,500 deaths/day from it. (Some 260 nonprofits have spent another $2.2 billion.) Why? She believes is because we don't focus enough on prevention, the direction that would benefit us most. Existing research shows we can prevent half of all cancers with what we already know. Yet, the National Cancer Institute is spending $2.3 billion this year on research to understand and treat cancer, and just $232 million on prevention. Meanwhile, tobacco companies still spend over $10 billion/year on cigarette advertising. Obesity, poor diet, and lack of exercise account for 30% of cancers, but high-fructose corn syrup, cornstarch, etc. receive more federal subsidies than fruits and vegetables.
Some of the most expensive new cancer drugs (eg. $100,000 for a single course) are hailed as 'lifesaving' breakthroughs, yet extend life by just weeks.
The American Cancer Society set a goal in 1996 of reducing the incidence of cancer by 25% by 2015. A 2007 progress report found a decline of just 0.6%/year, far less than required to achieve that goal. The cancer death rate has undergone a slightly greater decline - about 1%/year since 1990, on target for a 23% reduction by 2015. Meanwhile, the costs of cancer treatment (estimated $125 billion in 2010) are projected to hit $207 billion by 2020.
Part of the reason for under-emphasizing prevention is that prevention studies are slow and time-consuming.
There are 100 - 200 types of cancer, and new ones are still being discovered. Cancer does not occur spontaneously - a healthy cell's genetic material must first be damaged or altered. Some mutations are inherited, but about 90% are caused by DNA damage during one's lifetime. Environmental and occupational exposures can contribute. Fortunately, our bodies have built-in repair systems, but these become weaker after age 65. Viruses are linked to an estimated 15% of cancers - especial liver, cervix, leukemia. Medical radiation is also a significant cause of cancers, especially CT scans and scans improperly conducted with excessive exposure.
Most who have their cancer detected early are not helped - some cancerous cells progress no further, others are so aggressive they can't be stopped. We do need improved screening for breast cancer, as well as better tests for lung and ovarian cancers, and increased screening for colon cancer.
Another need - improved data-sharing. A 2008 survey found only 1.5% of 3,049 participating acute care hospitals had comprehensive electronic medical records, and another 7.6% with such capability in at least one clinical unit. The 'really bad news' is that there is very little interoperability. The National Cancer Institute has a similar problem assimilating and analyzing nationwide data.
We need more aggressive government action to ban BPA (plastic, can liners), perc (dry-cleaning solvent), and parabens (cosmetics, processed meats, some snacks, liquid dietary supplements). More education on the dangers of smoking and second-hand smoke, obesity (breast and colon cancers), transfats (heart disease, along with breast and prostate cancers), and red meat.
Vitamin D is GOOD. Statins in cholesterol-reducing drugs cuts colorectal cancer.

Powerful and Ambitious Look At How to End, Not Treat, Cancer

Dr. Cuomo's book, "A World Without Cancer", is a multi-faceted look at the US system "designed to study, diagnose, and treat cancer" and, as stated in the first sentence of this powerful and ambitious book, which "is fatally flawed." Dr. Cuomo poses a fundamental question about why there is not more emphasis on prevention (read: funding) rather than treatments.
As a seasoned radiologist, Dr. Cuomo offers a rare insight into both her own experiences with patients, as well as explaining, in lay terms, the "haunting statistics" of cancer despite the vast amount of research funding (she cites that the NCI has spent some $90 billion on research and treatment in the last 40 years) and science behind cancer. The book also contains countless stories of cancer patients which are "haunting" and all-too representative of stories that each of us have experienced directly, through loved ones, or through friends.
The book's structure makes it both a very human history of a very complicated disease and one that explains the basic science around it. It also suggests ways in which each individual can limit their potential cancer risk though behavioral and environmental changes - which should be essential reading for everyone. Throughout this fascinating discussion, Dr. Cuomo offers a way forward which is both inspiring and surprisingly hopeful. This is essential reading which hopefully will lead to needed discussions and debates about the next phase of the fight against cancer.
